Unless orders are paid in full at time of purchase, invoices are due and payable in full within 30 days following the invoice date. All claims for defective or damaged goods must be made within four (4) days after receipt of goods. Failure to notify NuMed shall constitute acceptance of work, waiver of defect, damage or shortage. Service charges of 1 % will be assessed per month , or as allowed by law, on all balances outstanding past specified credit terms. In the event of a delinquency, the applicant agrees to pay all reasonable attorneys fees and/or costs for the use of a collection agency incurred in the collection of the account. A $35.00 fee will be charged for each returned check.

In submitting this credit application, it is understood that NuMed Industries, LLC., is authorized to investigate the credit of the applicant. The applicant affirms that the financial ability exists to meet any commitments it has made, and will pay NuMed invoices according to the terms above.

Applicant also agrees that: 1. The extension of credit by NuMed to the applicant, serves as consideration for applicants agreements and that the account is payable in the offices of NuMed Industries, LLC. located in Palm Beach County at 1141 Holland Dr. Suite #30, Boca Raton, Fl 33487.

2. A current financial statement (audited if available) is attached.

3. Applicant grants NuMed Industries a security interest in all inventory furnished to applicant by NuMed and all proceeds from, to secure payment of all amounts which applicant owes NuMed. Applicant agrees to execute financial statements and such additional documents as NuMed may reasonably request to evidence the security interest and allow NuMed to perfect the security interest.

4. If any provision of this agreement shall for any reason be held in violation of any applicable law, governmental rule or regulation, or causes the agreement to be held unenforceable, the invalidity of that specific provision in this agreement shall not be held to invalidate the remaining provisions of this agreement.

5. THE LAWS OF THE STATE OF FLORIDA AND APPLICABLE FEDERAL LAWS SHALL GOVERN THE INTERPRETATION AND ENFORCEMENT OF THIS AGREEMENT. APPLICANT ALSO AGREES THAT THE COURT OF PALM BEACH COUNTY WILL HAVE JURISDICTION OF ANY DISAGREEMENT OR CONTROVERSY CONCERNING THIS ACCOUNT.

By completing and returning NuMed's application for credit, accompanied by a current financial statement, the applicant represents that all of the information contained in this application is true and correct and the applicant agrees that if any of the information becomes out dated or if the applicant learns of a possible or pending change of owner ship or management of it or any facility it will immediately notify NuMed.

The signor or applicant identified as proprietor, owner, and/or majority shareholder, authorizes NuMed Industries LLC., to verify this information by obtaining data from a credit reporting agency. The signor/applicant acknowledges that his or her individual credit history may be a factor in the evaluation of the credit history of the applicant and hereby consents and authorizes the use of a consumer report on the signor by NuMed as may be deemed necessary in its credit evaluation.
